As the anniversaries of Hurricanes Katrina and Rita approach, many of our fellow Louisianians and Gulf Coast neighbors are still in the process of recovery. The East Baton Rouge Parish School System is proud to have opened its doors and classrooms to children who have been unable to return to their former neighborhoods and schools.  We understand that many of our new students continue to be physically and emotionally impacted and that the anniversary of traumatic events often causes strong emotions to resurface.   Because the health and safety of all of our students, including our relocated students, is important to us, please use the information and links on this page if you need assistance in meeting this challenge.

MENTAL HEALTH ASSESSMENT FORMS

 

The following forms were developed through a partnership between I CARE, School Counselors and Health Care Centers in Schools of the East Baton Rouge Parish School System, along with a variety of mental health partners in the Greater Baton Rouge Area.  Due to the great number of students who were displaced and traumatized by the storm, a need arose to address the mental health issues of the victims.  After many hours of collaboration and research, these forms resulted.  Hurricane Intake forms were developed to fit the immediate needs following the hurricane and should be adapted as a non-evacuation version for use with all students for other traumatic experiences not related to a storm.  Please note that these forms are grade-level specific and the appropriate form must be used according to the age/grade level of the child being assessed.
